Palliative Care

FREE Holistic, Home-based
Palliative Care

Breede River Hospice provides free home based palliative care.

Palliative Care is an approach that improves the quality of life of patients and their loved ones facing problems linked to a life threatening illness. It starts with an assessment to draw up a care plan that focuses on the prevention and management of pain and any other symptoms linked to the illness. It is not just physical care but also looks to support patients emotionally, socially and spiritually.

Anyone with a life threatening / terminal illness may be referred to us. As a fundamental human right we believe every person deserves to be provided compassionate care that promotes quality of life.

We have a team approach to care which the patient is a part of.  Our doctor, nurses, social workers and volunteers are available to anyone referred to our palliative care programme..

Family Support Bereavement Councelling

Grief and loss can be overwhelming and support is needed for the healing of emotional pain and suffering.  Turning Compassion into Action our family support begins at the time of referral to Breede River Hospice. As a part of the patient’s assessment our social workers will meet with the family and loved ones to discuss their concerns and support needs.

Throughout the period of illness and into the time of bereavement we remain in close contact with the family and patient – providing whatever support and advice may be needed.  After the passing of a loved one, we remain committed to providing support to the family as they walk a journey of loss. Our bereavement service is provided for as long as it is needed.

Our psycho-social team is led by our professional social workers supported by volunteers trained in all aspect of grief and loss.

Palliative care is about turning compassion into action. It is the active management of an illness and can be provided from the point of diagnosis alongside other treatment. It provides a support system to help the family and other loved ones to cope during the patient’s illness and during bereavement.
